1. Accuracy. I confirm the information I provided is accurate, my pet's rabies vaccine is current, and I have disclosed any bite history, aggression, medical conditions, medications or behavior concerns.

2. Assumption of risk. I understand that caring for animals carries inherent risks, including injury, illness, stress, escape or death, even when reasonable care is taken.

3. Limit of liability. To the fullest extent permitted by law, and The Daily Paw are not liable for any injury, illness, loss, escape, damage or death of my pet, or any loss of or damage to my property, that occurs before, during or after care.

4. Emergencies. If my pet needs urgent medical care, I authorize to take my pet to a veterinarian, and I am responsible for all resulting costs. She will make a reasonable effort to reach me first.

5. My pet's behavior. I am responsible for any injury or damage my pet causes to people, other animals or property.

6. Home access. I authorize entry to my home during my booked dates and confirm I have the right to give that access.

7. Confirmation. A booking is not confirmed until confirms it with me.
